Intergenerational Prayer Partner Form (Adult)

This Form is to collect information from the adult prayer partners in our Intergenerational Prayer Partner Program. This information will help us select the right partner for you. (Please read the "Adult Prayer Partner Information and Code of Conduct" document before completing this form.)

For example, you may prefer to be partnered with an older child, because you believe you can better relate to them.

You may upload a picture of yourself for us to share with your prayer partner so that they know who is praying for them.
